Job Summary
Will oversee the Disease Management Program.
Responsible for department Protocol development and implementation per chronic disease.
Monitor program data to validate success metrics.
Essential Duties & Responsibilities Manages chronic illnesses, co-morbidities, and/or disabilities ensuring cost effective and efficient utilization of health benefits; conducts gap in care management for quality programs.
Provides assessment, education training to effective behavioral modification.
Responsible for the oversight of development planning, implementation, and evaluation of disease entities such as DM, Falls, COPD, cardiovascular disease.
Involvement in implementing cost effective, data based-health driven programs.
Coordinate a database of statistics and narratives for tracking progress and efficacy of treatments and program outcomes.
Oversee educational literature for patients and for staff.
Educate and train staff in new methods of approach for chronic disease management.

Supervisory Responsibilities Yes, oversees the Disease Management Coordinator Staff Training and Development Manage Employee Time & Attendance Critical Results Decrease re-admission in 3 disease entities.
Managing and monitoring productivity of coordinators on a monthly basis.
Producing quarterly re-admissions reports quarterly.
Education & Experience RN or Foreign Medical Graduate preferred.
3+ years' direct patient care experience Ability to work within a team and take direction.
Must be familiar with the EHR medical charting system.
Strong written and interpersonal communication skills Experience with eCW highly preferred Education Requirements Required/Preferred Education Level Discipline

Knowledge, Skills & Proficiencies Must be able to work in a face paced work environment.
Must have ability to multi-task (i.e., answer urgency calls and conduct home visits) Must have a valid driver's license and be able to drive to conduct field visits.

Must own exceptional leadership and organizational skills.
Must be competent to deal with people of different age groups.
Should have knowledge of computers and technology.
Excellent interaction skills are also compulsory.
Must have the ability to analyze and solve problems.
Possess excellent physical and emotional stability.
Should be willing in pursuing additional clinical and academic program.
Must have critical thinking, decision-making and observation skills, along with high energy level and good stamina.
Physical Requirements
This position works remotely conducting patient visits in the field.
The employee is required to work at a personal computer as well as be on the phone for extended periods of time.
Must be able to stand, sit, walk, and occasionally climb.
The incumbent must be able to work extended and flexible hours and weekends as needed.
Physical demands include the ability to lift to 50 lbs.
