Our Licensed Practical Nurses assist with all (home based) skilled nursing care based off the plan of care for patients located within the Laurel, Lakeside, Richmond, Glen Allen area . The Licensed Practical Nurse will effectively interact with patients, caregivers, family members and other interdisciplinary co-workers to provide skilled nursing care to patients on an intermittent basis.

Required Qualifications: Graduate of an approved school of professional nursing Current and valid Virginia LPN License Passionate and motivated to provide care within a patient's home. Ability to exercise initiative and independent judgment. Comfortable working collaboratively in cross-functional teams Ability to communicate effectively, both verbally and in writing, with all levels of an organization Ability and willingness to travel within a given territory. Willingness to work with multiple patients in varying home locations. Preferred Qualifications: Two (2) years' home health nursing experience, preferred. Previous experience with Home Care Home Base EMR system strongly preferred. Wound Care experience or Certification strongly preferred.
