We take pride in not only providing excellent care to our patients but also creating a positive team environment with employee support.We provide· Competitive Pay· Team Events· Recruitment Incentive Program· Continuing Education Training· Employee Recognition Programs· Performance Incentives· Family Team Environment*Job Summary:*The home care nurse (LPN) will administer nursing care, under the supervision of a registered nurse, for clients in their place of residence, will coordinate care with the Home Health LVN interdisciplinary team, clients, and their families, and the referring Agency.*Job Responsibilities:*· Under the direction of the RN, assist in identifying the client's physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations.· Participate in planning and implementing care in conjunction with the RN, in accordance with the Plan of Care (POC). Assists clients in the recovery or maintenance of life functions; prevent illness and relapse of illness.· Assure the continuity of care through the delivery of quality patient care.· Provides effective communication to clients, their families, team members, and other health care professionals, and regular participation in Case conferences/Team Meetings.· Monitors assigned cases to ensure compliance with the requirements of third-party payers.· Completes appropriate documentation in a timely manner to assure compliance with company policy.· Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency.*Experience:*· 1-year experience as an LVN/LPN in a clinical care setting.· Preferred, home health experience..*Home health Nurse Requirements:*· A current license that is in good standing to practice as an LVN/LPN in any/all applicable states.· Current CPR certificate.· Excellent observational and problem-solving skills.· skills as defined as generally accepted standards of practice.· The ability to follow instructions but also to act independently if necessary.· Excellent report writing skills.· Active listening skills and empathy.· Physical and mental strength.· A valid driver's license and reliable transportation, insurance required.*Education:*· Completion of an accredited Vocational Nursing Program.#INDZYJob Types: Full-time, Part-time, PRN, Per diemPay: From $25.00 per hourBenefits:* 401(k)* Dental insurance* Flexible schedule* Health insurance* Life insurance* Paid time off* Vision insuranceMedical Specialty:* Home HealthExperience:* Home health: 1 year (Preferred)* Kinsser/WellSky: 1 year (Preferred)License/Certification:* LPN/LVN (Preferred)Ability to Commute:* Taos, NM (Preferred)Work Location: In person
