Position Purpose / Summary The Licensed Practical Nurse (LPN) is normally the first clinical staff to provide preliminary medical services to patients of Primary Care Associates. This position utilizes strong communication skills to gather information and data from the patient that will provide the physician, nurse practitioner or physician assistant with the information necessary to begin the treatment and care of the patient. The CMA also is the initial clinical contact for any patient seeking prescription refills outside the designated appointment or patients contacting the office with medical questions. This individual must possess a strong desire for excellent patient care and best outcomes. The LPN is responsible for taking patient orders from the provider and making sure that all requests for the patient are organized and processed promptly. This position collaborates with other ancillary departments, other medical facilities and other medical offices for communications needed for patient care.
\n ResponsibilitiesPerforms nursing procedures under the supervision of a licensed physician, nurse practitioner or physician assistantEscorts patients to exam rooms, interview patients for visit informationObtains patient vital statistics and records them into the EMRDocuments the patient's chief complaint, family social history, personal history, medications and any other pertinent data into the EMRPerforms EKG, PFT, injections, Holter Monitor set ups, blood pressure checks and other duties as the legal scope of practice allowsProvides patient education and instructions as directed by the providerAnswers and documents patient telephone calls regarding prescription refill requests theninforms the provider and notifies the pharmacy and patient after determination is made by the providerAnswers and documents all patient calls in the EMR, conveys the information to the provider and responds promptly to the patient per the direction of the providerTriage and process messages from the patients, front office staff, other medical offices and other care facilitiesMaintains logs and required checks (i.e., vaccines, refrigerator temperatures, emergencymedications, controlled medications, etc.)Planning and managing patient care according to each patient's needsInterviewing patients and recording their medical historyObtaining patient vital signs and escorting patients to their roomsAdministering injections of medicationsVerifying that patient charts are updated promptly and accuratelyObserving and recording patients' conditions and reactions to medications and treatments Knowledge & EducationGraduation from an accredited entity is preferred. National / State certification as a Licensed Practical Nurse (LPN) is required.
