Licensed Practical Nurse (Lpn)

We are currently looking to fill 32-hour per pay period LPN positions on our 2 p.m. - 10:30 p.m. evening shift and our 10 p.m. - 6:30 a.m. night shift (both shifts are eligible for a $1.60 shift differential). A relocation assistance bonus is available for eligible candidates. Job Summary: The Licensed Practical Nurse shall be accountable and responsible for performing tasks and responsibilities within the framework of case finding, health teaching, health counseling, provision of supportive and restorative care, administering medication(s), and provide nursing treatment under the direction of a registered nurse, licensed physician or other health care provider. The Licensed Practical Nurse shall be responsible for: Monitoring that Plan of Care is being followed by clinical staff under their supervision; Supervise certified nurse aides or unlicensed assistive personnel Provide supervision, coordination, and evaluation of assigned unit health care team and services. Unit Description: The Alice Center's Skilled Nursing Facility offers 24-hour nursing services and a full range of rehabilitation services in a caring, home-like environment.  The Alice Center can serve up to 135 individuals in need of long-term or short-term care.  We provide the highest quality skilled nursing care in an environment that promotes privacy and dignity. The Alice Center is divided into six separate "neighborhoods" that each feature their own home-like kitchens and dining rooms, large living rooms, and ample social spaces.  Each resident room is either private or semi-private and contains a private bathroom and shower. Education: Currently licensed as a Practical Nurse in New York State.
