The Intermediate Care Facilities Program (ICF) runs 6 residential homes that caters to adults who are diagnosed with an intellectual disability and unable to care for their own basic needs.
Our team provide our residents with support in residential habilitation, integration into the community and activities of daily living.
The main goal for this program is to encourage growth for all individuals we serve toward living more independent lives.
The Individualized Residential Alternative (IRA) Program operates 12 residential homes that caters to adults who are diagnosed with moderate to severe developmental disabilities.
The hallmark of the IRA program is our People-Centered Approach, where the individual is the primary decision maker regarding their life and treatment.
Our residents go on outings, play sports, conduct household chores, work and have fun!
General Statement of Duties:
Under the Supervision of the Director of Nursing, the Licensed Practical Nurse is responsible for the overall oversight and support of the healthcare needs of the people we support in a residential setting.
This will also include additional support to the residential staff in the programs we serve.
The Licensed Practical Nurse will work in conjunction with the Director of Nursing, Registered Nurse and the Residential Administrative team to ensure the delivery of the highest quality of integrated health and continuity of care to the people residing in our residences.
The qualified candidate must be able to work in partnership with all personnel within the organization to ensure the highest quality of clinical services for the people we support in a manner that is consistent and aligned with the agency mission and vision.
Essential Job Functions
The Licensed Practical Nurse serves as a member of the Interdisciplinary Team and primary functions include: Assist with delegated nursing functions within the scope of practice of an LPN under OPWDD (See attached guidance) and as per established procedures, policies, guidelines and standards as observed by the Registered Nurse.
Assist in data collection on individuals within assigned caseload including, but not limited to, lab results, diagnostic testing, BM data.
Assist with performing tasks and responsibilities related to individual's medical care and environment in a safe manner.
Contribute to the development of individual's plans and assist in identifying areas of need.
Assist with adherence to physician orders accurately.
Assist in observation and evaluation of nursing care administered by Direct Support Professional Team Members.
Assist with documenting pertinent observation and intervention as required by policy and regulation: including, but not limited to, AMAP observations, BM protocols, feeding protocols, diabetic protocols.
Assist in maintaining effective communication with staff, physicians and Registered Nurse.
Communicate information regarding individual's medical status and overall needs to the Registered Nurse, physicians and pertinent team members.
Assist with oversite of medical appointments when necessary and/or requested by Nursing Administration.
Assist with oversite of reviewing physician summary forms to ensure information is complete, accurate, and documented in the individual's medical record, including any indicated test results and recommendations for follow up as well as resolution of a medical issue.
Work in partnership with the clinical/program management to ensure that provider visits are followed upon and staff are securing follow up appointments as indicated.
Assist with ensuring Emergency Room instructions and/or treatment is followed up and documented accordingly.
Ensure staff are educated as needed on proper follow up and treatment.
Emergency Room Documentation should be obtained by staff upon discharge.
Assist with providing staff trainings as delegated and observed by the Registered Nurse.
Provide coverage in emergency situations until relieved of the responsibility, in the event of unusual circumstances this includes but is not limited to pandemics, extreme staffing shortages, emergency situations defined as an individual emergency medical visit, unannounced arrival from home visit or day program until relief staff arrive on shift.
Perform additional nursing responsibilities, as assigned.
Ensure compliance with regulatory standards.
Observe agency policy and procedures regarding facility security and proper identification of authorized personnel.
Ensure appropriate protective and preventative measures are followed by staff and individuals in the control of communicable and infectious diseases along with the identification, assessment, and reporting of the same.
Attend Inter-Disciplinary Team Meetings in assigned caseload, division meetings and staff meetings.
Duties and Responsibilities
The Licensed Practical Nurse will: Adhere to a flexible schedule that reflects, and is based on, the needs of the program.
If warranted, change in program location and oversight may occur.
Ensure adequate support is provided to the individuals based on Life Plans, Behavior Support Plans, and Nursing Care Plans.
Ensure adequate support is provided to the residential staff and programs we provide service to.
Required Qualifications Valid NYS LPN license Valid NYS driver's license Preferred Qualifications Experience working with the differently-abled population preferred Knowledge of OPWDD state and federal regulations preferred.
Ability to manage multiple priorities and ability to be flexible to meet the needs of a diverse population.
Strong verbal and written communication and organizational skills.
