We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our telehealth team in-office. Responsibilities:
As an LPN in our telehealth setting, your duties will include, but are not limited to:
Patient Assessment & Monitoring:Conduct in-office patient assessments, gathering health histories, vital signs, and symptoms to assist in creating comprehensive care plans.Monitor patient conditions remotely via telehealth platforms, tracking key health indicators and ensuring that patients are receiving appropriate interventions.Maintain accurate and detailed patient records in electronic health systems, ensuring up-to-date and organized documentation.Patient Care & Education:Provide patients with instructions, educational resources, and guidance on managing their health, medications, and lifestyle modifications.Deliver culturally competent care, especially to Spanish-speaking patients, ensuring clear understanding of their treatment plans and follow-up requirements.Address patient questions and concerns regarding their care, medications, and overall health, providing exceptional service both in-office and through virtual consultations.Collaboration & Communication:Work closely with physicians, physician assistants (PAs), nurse practitioners (NPs), and other healthcare providers to coordinate care and develop tailored treatment plans.Participate in daily team meetings to discuss patient progress, make recommendations, and adjust care plans as needed.Coordinate with external healthcare services, such as pharmacies and labs, to ensure that patients receive necessary prescriptions and tests.Telehealth Technology Utilization:Use telehealth platforms in-office to facilitate patient consultations, ensuring smooth and efficient virtual interactions.Troubleshoot any technical issues with patients, guiding them through the telehealth process to ensure effective communication and care.Stay up to date with advancements in telehealth technology and provide feedback to improve patient care delivery and virtual processes.Compliance & Quality Control:Adhere to all healthcare regulations and standards, including HIPAA, ensuring patient privacy and confidentiality at all times.Maintain accurate and timely documentation for all patient interactions, treatments, and follow-ups, ensuring compliance with legal and billing requirements.Ensure that all in-office and telehealth practices meet industry standards for quality care and patient safety. Requirements: Education & Licensing:Valid and current LPN license in good standing (required).Completion of an accredited LPN program.Experience & Skills:Proven experience in a clinical setting, preferably in a telehealth or virtual healthcare environment.Fluency in Spanish (required) to provide care to Spanish-speaking patients.Experience using telehealth technology or a strong willingness to learn and effectively use telehealth platforms.Strong communication skills (verbal and written) to interact with patients, healthcare teams, and external providers.Personal Attributes:Compassionate, patient-centered care approach, with a commitment to helping patients navigate their healthcare journey.Excellent organizational skills and attention to detail, ensuring accurate documentation and effective follow-up care.Ability to work well independently while collaborating as part of a dynamic healthcare team in a fast-paced, in-office environment.
