WHAT WE'RE LOOKING FOR: Degree from an Accredited LPN program or RN License Must maintain a valid nursing license in the State of Michigan Have a good internet connection in your designated work area at home Qwerty typing skills Proficiency in conducting patient assessments and knowledge of medical terminology.
Excellent communication and interpersonal skills.
Ability to work independently and manage time effectively in a remote setting.
Compassionate and patient-centered approach to healthcare.
WHAT YOU WILL BE DOING: Conduct Phone Assessments: Perform detailed patient assessments via phone to gather comprehensive health information, evaluate symptoms, and determine the level of care required.
Patient Education: Provide clear and accurate information to patients about their health conditions, treatment plans, and preventive care practices.
Care Coordination: Collaborate with a multidisciplinary team to coordinate care and follow-up for patients based on assessment findings.
Documentation: Maintain accurate and timely documentation of patient interactions, assessments, and care plans in compliance with healthcare regulations and organizational policies.
Triage and Referral: Identify patients who require further medical evaluation or intervention and facilitate appropriate referrals to healthcare providers or emergency services.
Patient Advocacy: Advocate for patients by addressing their concerns, answering questions, and ensuring they have access to necessary resources and support.
Follow-Up: Conduct follow-up calls to monitor patient progress, adherence to treatment plans, and address any ongoing health concerns.
Professional Development: Stay up-to-date with the latest nursing practices, telehealth technologies, and healthcare regulations through continuous education and training.
