Participate in interdisciplinary care plan conferences and contribute to the development of care plans for residents. Establish specific social action plans and realistic goals for each resident consistent with medically related social and emotional needs. Provide individual and group support for residents and families during times of adjustment, crisis, or particular need. Orient residents and families to the facility environment and assist their adjustment through individual and group programs. Coordinate discharge planning with staff, residents, and families. Coordinate social services in-service and educational programs for staff. Promote facility programs to ensure priority consideration of referrals by doctors, hospital discharge planners, community agencies, churches, etc. Requirements: A Bachelor's Degree in social work (BSW) or a secondary education in social services with certification as a social worker designee may be substituted as appropriate. Effective interpersonal and communication skills are required. Long-term care experience is preferred.
