Licensed Social Worker (Lsw)

We are looking for an experienced LSW with Skilled Nursing Facility experience to join our dynamic interdisciplinary team at a 124-bed facility in Amesbury, MA. Experience with MDS, Care Plans, discharge planning, leveling is required. Looking for a motivated LSW that can take charge and demonstrate creative, customer service oriented skills functioning independently, as well as part of a team, to achieve positiveResponsibilities- Conduct patient assessments and develop individualized care plans.- Provide behavioral therapy and counseling to patients.- Maintain accurate medical documentation and records.- Collaborate with interdisciplinary teams for discharge planning.- Utilize premium skills such as psychotherapy and motivational interviewing.- Ensure patient confidentiality and compliance with HIPAA regulations.Job Type: Part-timePay: $37.00 - $42.00 per hourExpected hours: 16 – 24 per weekMedical Specialty:* GeriatricsSchedule:* 8 hour shift* Day shiftExperience:* Social Work: 1 year (Preferred)License/Certification:* LSW (Required)Ability to Commute:* Amesbury, MA 01913 (Required)Work Location: In person
