*Licensed Social Worker* for TruCare Living Center - PalestineHourly pay based on Experience $26.00 to $30.00 per hour based on experience for Texas Licensed Social Worker with at least one year of experience in Social Work._TLC-Selma_ is a Beautiful Skilled Nursing Facility serving Short Term Rehab and Long Term Care residents. We are so please to be able to provide such a lovely environment for our elders.Duties of Social Services:- Admissions Assessments, BIMS, and PHQ-9- Schedule Care Plan Meetings- Provide Discharge Planning to Rehab residents returning home- Serve on Quality Assurance Committee- Advocate for residents and manage the grievance process- Complete Social Services section of MDS AssessmentSkills:- Must be a Texas Licensed Social Worker with a degree in Social Services or a related healthcare degree- Must have worked for one year under the direction of a Qualified Social Worker- Use PointClickCare electronic healthcare documentationThis position offers the opportunity to make a meaningful impact on the lives of individuals in need.
