Facility Location: Hospital Coverage- St Bernadine; 2101 N Waterman Ave, San Bernardino, CA 92404Clinic-401 E Highland Ave, San Bernardino, CA 92404 (located right behind the hospital) Setting: Clinic and Hospital FTE: 1 Providers Needed: 1 Duties: inpatient consults, outpatient/office consults, follow up appointments, surgeries, call coverage Required Procedures: CABG, valve surgery, TAVR, lung/pleura surgery EMR: Cerner Patients per Day: Varies 2-4 a day Support Staff: 4 APPs and 2 other surgeons Reason for Coverage: Physician on medical leave Deal Breakers? Unreliability or tendency for last minute cancellation Travel Preferences: Preferred Lodging: Hampton Inn San Bernardino or nearby the hospital (within 15mins of hospital) Air Travel: Local, no flights HIGHLY PREFERRED Credentialing Information: Timeframe: 90 days Requirements: Emphasize to candidate that credentialing takes at least (90) once a completed packet is received DHMF packet required; St. Bernadine packet required if practice includes inpatient work &/or Hospital credentialing Credentialing with other facilities? St Bernardines Hospital and DHMF Billing enrollment: Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?
