POSITION SUMMARY
LPNs will deliver care throughout Wellbridge and are responsible for the day-to-day nursing requirements of a licensed inpatient medically supervised stabilization and withdrawal program, rehabilitation program and reintegration program.
Carries out the Wellbridge vision, values and overall strategic planning, clinical management, patient care, budgeting, financial management and quality management.
Promotes service standards and creates a patient-focused caring environment while addressing and meeting the individual needs of all patients.
Fosters an environment of collaboration and respect with physician colleagues, nurses, therapists and other healthcare team members.
RESPONSIBILITIES
Clinical Provide on-site nursing support to all treatment programs Assists healthcare team with inpatient services including interviewing patients, vital signs, height and weight, data entry into an electronic medical record Performs select clinical tests including, but not limited to; urine drug screening, pregnancy tests and blood glucose monitoring Manages and operates equipment safely and correctly.
Demonstrates appropriate hand washing technique to insure proper infection control and follows universal precautions.
Administers and documents medication under physician's order and follows protocols for safety and reduction of medical error.
Participates in the controlled substance counts per protocol Assists physicians with office-based procedures such as administration of intramuscular injections Observes, records, and reports changes in patients' condition.
Organizes exam rooms, including general cleaning, upkeep, and re-stocking of supplies.
Fields clinically-related telephone calls and responds Maintain understanding of current treatments, knowledge, and practices in addiction care.
Collaboration Support a culture of high quality of care across organization.
Work collaboratively with the Director of Nursing and other nursing colleagues to develop personalized treatment plans Support efforts of Wellbridge research team to implement program improvement Manage and promote relationship with academic and research institutions to promote addiction research and training.
Facilitates the effective provision of support services essential to team-based patient care Provide crisis support to other clinicians and patients, as needed Supervision and Support Model the Wellbridge philosophy of patient-centeredness, empowerment, and cultural proficiency and seek supervision from clinical staff Maintains a safe and caring environment for patients that is conducive to positive health teaching, and maintenance; patient and family expectations of care Recognizes the need for patient care to be delivered in a culturally sensitive manner and models sensitivity to all staff Planning and Coordination Participate in regular clinical team meetings Work closely with the clinical and advocacy teams to coordinate services to patients Works with leadership team to counsel staff as needed Makes recommendations for growth and development of organization Responds in a timely fashion to staff concerns and functions proactively to retain staff Assist with community outreach/presentations, as needed EXPERIENCE AND QUALIFICATIONS
Required Qualifications Graduate of an accredited LPN program Current license to practice as an LPN in New York State.
Current certificates for BLS and Infection Control Clinical competences in phlebotomy and point-of-care (POC) testing Familiarity with all relevant healthcare standards: OASAS, ASAM, JCAHO, etc.
Ability to assess, plan, implement and evaluate nursing care according to individual needs.
Demonstrate an understanding of clinical risk Demonstrate consistent professional conduct Demonstrate effective communication and interpersonal skills Preferred Qualifications Clinical experience providing treatment for addiction and co-occurring mental health disorders Experience with stabilization and withdrawal management settings Experience in an addiction rehabilitation and/or residential environment Experience with therapeutic groups (preferably addiction-oriented), therapeutic communities, and inpatient care Knowledge of medication assisted treatment (MAT) and how behavioral interventions can support and improve outcomes for MAT Experience working as part of a multi-disciplinary team Demonstrated ability to create a positive, respectful, collaborative, and team-focused environment
