Allegria Village is hiring for a FULL-TIME, Assessment Nurse (LPN). The shift would be MONDAY through FRIDAY 9am - 5pm. Education/ Certification: • RN (BSN preferred) in compliance with regulations of the state.
• Supervisory experience in team leader role/educator in LTC.
• CPR certified
• Infection Control and P revention experience, required.
Job Responsibilities:
• Receive and Review Discovery form and documentation provided by Sales and Marketing
Department via assessment request email. After reviewing documentation admission assessment
must be scheduled and completed asap.
• Once assessment is complete findings are to be reviewed with Director of Nursing for
determination of admi ssion. All denials must be reviewed with Regional A dmissions C oordinator
before they are processed. **• A ssessments are to be completed every 6 months and with a change in resident condition.
• Responsible to follow up with families/POA/Resident with all changes in level of care.
• Responsible to notify billing with any changes in care levels for correct billing status (increase
or decrease in care levels).
• All assessments should be kept by Nurse Liaison in binders; GSE binder, GSP binder, and
HCP binder. All three binders must be brought to LOC meeting weekly.
• RN must review and sign each assessment completed.
• W ill maintain documents in an organized manner.
• Will document communication with family/POA, MD and Resident
• Other duties as assigned.
