Responsibilities
• Educate patients on self-management and goal setting• Implement and improve the Plan of Care• Utilize behavior change techniques to coach and assist patients• Conduct Transitional Care Management activities for high-risk patients• Reduce care gaps and encourage preventive care• Connect patients with community resources
Requirements
• Fluent in English• Strong communication and telephonic skills• 5+ years experience as a Licensed Practical Nurse• Proficiency with electronic health records and web-based applications• Availability to make calls on weekdays between 9am-7pm

Benefits
