Job SummaryThe Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices. The Licensed Practical Nurse (LPN) assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/ providers , registered professional nurses and multidisciplinary care team members. A Licensed Practical Nurse (LPN) can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse (LPN) in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. A Licensed Practical Nurse  will follow the mission, vision, I-CARE values, policies and procedures of Mohawk Valley Health System.Core Job ResponsibilitiesGather patient measurements and collect patient dataRecord and report patient findingsPrepare patient for patient examination and treatmentAssistswith examination and treatmentsPerform tasks within the framework of supportive care, under the clinical direction of licensed health care provider or registered professional nurseSupport self-care , self- management and the established plan of care.Utilize standard precautions and practice at all timesMaintain equipment, supplies, inventory, and a safe and clean work environmentMaintain technical knowledge by attending educational workshops, mandatory in-services; reviewing publications and participating in department committee meetings or work groupsEngage in quality improvement initiatives including continuing education, participation in system-wide and department specific teamsMaintain a cooperative relationship among the healthcare team by effectively communicating informationRespond to patient requests, communicates effectively to the licensed health care provider who can fulfill the patient requestEducation/Experience RequirementsRequired: Graduate of an accredited school of licensed practical nursing programPreferred: Bachelors or Masters of Science in Nursing from accredited nursing programLicensure/Certification RequirementsRequired: Current New York State licensure as a Licensed Practical Nurse; Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire dateDisclaimerQualified applicants will receive consideration for employment without regard to their age, race, religion, national origin, ethnicity, age, gender (including pregnancy, childbirth, et al), sexual orientation, gender identity or expression, protected veteran status, or disability.Successful candidates might be required to undergo a background verification with an external vendor.Job DetailsReq Id  89739 Department  MG - PRIM - WASHINGTON MILLS Shift DaysShift Hours Worked  8.00FTE 1 Work Schedule  HRLY NON-UNION-8 HREmployee Status A1 - Full-Time Union Non-UnionPay Range $18-$29 Hourly
