Position Requirements and Specifications:
Education: Current/Active License as an LPN in the State of Minnesota required.
Current BLS
Certification or ability to obtain within 30 days.
Preferred Experience: One year assisted living or long-term care experience preferred.
Previous
experience working with a senior population preferred.
Previous experience working with an
Electronic Medical Record.
Previous experience caring for residents with dementia.

Major Position Accountabilities:
A.
Assume role of the charge nurse to supervise resident care as assigned.
1.
Monitor Resident Assistant performance, ensures completion of resident cares, and
medication administration.
2.
Provides input in the development of the Resident Assistant's performance
3.
evaluations.
4.
Notifies Director of Health Services or designee of Resident Assistant performance issues in
a timely and professional manner.
5.
Assist in the training and supervision of Resident Assistants, including on- time education.
6.
Demonstrate a professional demeanor and boundaries with residents, visitors, and
providers at all times.
B.
Responsible for direct care, monitoring and evaluation of services provided to residents.
1.
Maintain resident records and other documents according to department policy and
procedures.
2.
Provide monitoring of new and existing resident's medical conditions and functions and
updates the RN regarding changes of condition.
3.
Provide appropriate nursing intervention for reported and/or observed instabilities in the
resident's medical condition or changes in their functional status.
4.
Receives, transcribes, and completes laboratory orders.
Notifies the attending physician of
lab results.
5.
Provide medically necessary treatments as ordered by the physician or nurse practitioner.
C.
Assist the Director of Health Services with medication systems management and compliance.
1.
Completes medication set-ups.
2.
Sets up insulin for administration by assigned staff.
3.
Monitors Resident Assistants in performance and compliance standards.
4.
Checks in medication when received from the pharmacy.
5.
Audits MARs and Physician Orders Sheets to ensure the orders are complete and accurate.
Either hard copy or electronic version.
6.
Transcribes physician orders to the designated MAR and updates the resident's record
accordingly.
D.
Facilitate a verbal and written communication system with Health Services Director, Complex
Supervisor, caregivers, physicians and family members to ensure effective, continuous resident
care and high levels of customer satisfaction.

Changes in resident conditions are communicated to on-call RN, Complex
2.
Supervisor, Supervisor in a timely manner.
3.
Contacts the physician/NP per established procedures for changes in resident condition.
E.
Ensure the safety and well-being of residents at all times.
1.
Monitor to ensure resident rights are maintained and notify the immediate supervisor of
issues in a timely manner.
2.
Knowledgeable of the Vulnerable Adult Prohibition policy and reporting requirements when
there is a potential brief of the V.A.
Law.
F.
Collaborate with the Corporate Quality Improvement staff and participates in quality
improvement initiatives.
G.
Support the marketing efforts of the community and its programs.
H.
Perform other duties as assigned by the Director of Health Services or designee.
