As an LPN you will:
Perform patient observation, data collection related to patient care, administers medication, treatment and procedures according to the physicians' orders and resident care plan.
Communicates patient findings to the supervisor.
Accountability for facilitating, communicating and collaborating with the healthcare team to identify and meet the physical, emotional and spiritual needs of the patients.
Oversee care provided by CNAs, Care Partners, QMAPs and/or other support staff, while also promoting the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act.
Demonstrate personal accountability for relationship-based care, organizational mission, and core values.
Qualifications:
In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:
One year of previous LPN experience, preferred
Requires knowledge in nursing and person-centered care concepts
Ability to demonstrate competency in the provision of individual care of patients based on the patients' assessed age and developmental needs.
HS GED
Current state LPN license for applicable state of employment or current LPN license from a participating state in National Compact Licensure (Colorado only)
BLS
Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
This home health role will need to be open to cover any territory in the Lakewood.
