As an LPN, you will play a vital role in providing excellent healthcare services to our participants. Working in a clinic setting, you will collaborate with registered nurses and physicians to deliver high-quality care and support to patients enrolled in our Program of All-Inclusive Care for the Elderly (PACE).
Midland Care's Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement to stay in their own homes and communities.
Employment Status: Full-Time
Pay Rate starting at $26/hr (Based on experience)
$2,500 Sign on Bonus Essential Duties & Responsibilities Assures excellent customer service is provided to all participants.Completes nursing responsibilities including but not limited to monthly medication reconciliation, quarterly medication review with participants, and timely completion of required Functional Capacity Screens and Health Services Plans as designated by regulation.Promotes and maintains the health of participants through evaluation of care needs, and provides teaching and instruction to meet care goals.Provides skilled nursing care, by following identified protocols, infection control standards and prescribed treatments.In conjunction with RN, identifies priorities on a daily basis and provides needed confirmation of critical activities before leaving the site.Assists in the evaluation of medication compliance and the effectiveness of prescribed medications.Assists in the transcription of medication orders and communication with pharmacy providers.Attends and participates in daily meetings, and scheduled participant care meetings including IDT as requested.Confers with the Interdisciplinary Team for Plan of Care adjustments and enters changes in Electronic Medical Record as agreed upon by the team.Maintains confidentiality of all participant events, discussions and records.What We Provide for You: Health insurance options for employee and dependentsFree Employee Assistance Program (EAP) for employees and dependentsDental, vision, and prescription drug plansFlexible Savings Accounts (FSA)Aflac Supplemental InsurancePaid Time Off (PTO)Extended Medical LeaveBereavement Leave (up to 10 paid days)Holiday pay (six holidays per year)403(b) ERISA Retirement Plan, match providedFree group life and AD&D insurance of $20,000Optional voluntary life at group ratesOptional long term care insurancePaid monthly volunteer time at non-profit organizationsOn-site education center to provide professional development and CEU creditsOn-Demand Pay OptionQualifications Required: Current unrestricted State of Kansas License to practice as a Licensed Practical Nurse; Valid Kansas Driver's License. Must possess valid Kansas Driver's License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by Midland policy.
