Lpn (Licensed Practical Nurse)

*Job Overview*


We are seeking a dedicated Licensed Practical Nurse to join our home care team.
The ideal candidate will provide quality nursing care under the direction of registered nurses.
Care setting is in a client's private home.




*Duties*


- Implement and coordinate patient care plans


- Administer medications and treatments as prescribed by healthcare providers - including tube feeding.


- Monitor and assess patients' conditions


- Assist with mobility / turning patient.


- Assist client who is immobile with bed baths and personal hygiene including changing of condom catheter.


- Maintain accurate medical records




*Experience*


- Prior experience in infection control, wound care and tube feeding preferred


- Familiarity with private home care setting (working in a client's home)


- Plus: Experience using a hoyer lift, working with a bed bound patient.
Wound care experience a must.




*Schedule*




*Tuesday, Wednesday and Friday, 11am - 3pm.
12 hours per week with possible increase .
