The Lung Cancer Screening Navigator focuses on achieving healthcare outcomes within effective and realistic time frames. This position facilitates patient follow-up through coaching, information transfer and communication to improve continuity of care both within the organization and in conjunction with referring providers. This role serves as the Lung Cancer Screening Program (LCSP) expert and administrator by collecting, tracking, maintaining, and reporting all relevant program data. The Navigator is responsible for assisting with coordination of clinical involvement, collection and disbursement of data and information, as well as supporting the needs of patients and their families who are facing a possible cancer diagnosis, are newly diagnosed with lung cancer, and those at increased risk and being monitored for potential development of lung cancer.
Minimum Education Bachelor's degree preferred Completion of an approved and accredited educational program for the licensed profession Minimum Work Experience 4 years' experience in their Clinical profession Prior experience guiding, mentoring, or educating others preferred. Required Licenses/Certifications Holds current and maintains VT State Licensure. Radiologic Technologist, or Respiratory Therapist BLS Certification through American Heart Association. Required Skills, Knowledge, and Abilities Ability to apply critical thinking to all aspects of patient care. Conflict resolution skills. Relationship-building and influencing skills.
