JOB TITLE: Charge Nurse-LPN/LVN/RN

DEPARTMENT: Nursing

REPORTS TO: Director of Nursing Services

PURPOSE OF JOB POSITION

The primary purpose of this job position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.

DELEGATION OF AUTHORITY

Charge Nurses are delegated the administrative authority, responsibility, and accountability necessary for carrying out the assigned duties.

DUTIES AND RESPONSIBILITIES

§Provide direct nursing care in accordance with current rules, regulations, and guidelines that govern the long-term care facility and patient centered care plan to attain and maintain the highest practicable level of resident functions.

§Admit, transfer and discharge residents in accordance with established procedures.

§Receive and record telephone orders from physicians on the Physician's Order Form, then transcribe physician's orders as applicable to Medication Administration Record, Treatment Records, etc.

§Prepare and administer medications as ordered by the physician.

§Administer other professional services such as catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking vital signs, care for the dead/dying, etc.

§Order prescribed medications, supplies, and equipment as necessary, and in accordance with established policies and procedures.

§Participate in the development, implementation and evaluation of patient centered care plan.

§Notify the resident's attending physician and representative when there is a change in the resident's condition and when resident is involved in an accident or incident.

§Make rounds with physicians as necessary.

§Arrange for diagnostic and therapeutic services, as ordered by the physician.

§Provide education and training to residents, resident representatives or caregiver.

§Coordinate with other resident services to ensure that the resident's total regimen of care is maintained.

§Direct and monitor the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.

§Participate in the development, implementation, and maintenance of the infection prevention and control program including antibiotic stewardship.

§Ensure that the assigned work areas (i.e., nurses' stations, medication preparation rooms, etc.)
are maintained in a clean and sanitary manner.

§Document nurses' notes in a manner that reflects the resident's condition, the care provided and the resident's response to the care.

§Report problem areas to Nurse Supervisor and/or Director of Nursing Services.

§Participate in the orientation of new charge nurse to the unit, and to his/her job position and duties.

§Inform the Nurse Supervisor and/or Director of Nursing Services of staffing needs when assigned personnel fail to report to work.

§Attend and participate in continuing education programs designed to keep abreast of changes in profession and to maintain license on a current status.

EDUCATION REQUIREMENTS

§Must be a graduate of an accredited college or university.

§Must possess a current, unencumbered, active license to practice as a Licensed Vocational Nurse/ Licensed Practical Nurse or Registered Nurse in this state.

§Must possess a current Basic Life Support [BLS] certificate.

PHYSICAL AND SENSORY REQUIREMENTS

Physical and sensory requirements of this position, with or without the aid of mechanical devices include: Walking, reaching, bending, lifting, grasping, fine-hand coordination, pushing and pulling, ability to communicate with residents, ability to distinguish smells and temperatures, ability to read and write, and the ability to remain calm under emergency situations while dealing with multiple tasks.

EXPERIENCE

No experience is necessary.
New graduates from accredited nursing college or university with current valid nursing license is acceptable.
On-the-job training is provided.
