Job Description Shift: Various Work location: Onsite; West Hollywood, CA Compensation: $35 to $41 per hour Duties and Responsibilities Conduct patient assessments to determine their medical needs, social situations, and support systems.
Monitor patients' conditions and progress, ensuring that care plans are adjusted as needed based on changes in health or circumstances.
Collaborate with physicians, nurses, social workers, and other healthcare professionals to create comprehensive care plans tailored to the patient's needs.
Coordinate referrals to specialists, physical therapy, or other external healthcare services, as well as assist in securing community resources (e.g., home care, transportation).
Ensure that all healthcare services, treatments, and care plan interventions are provided effectively and in a timely manner.
Advocate on behalf of patients, providing education about their conditions, treatment options, and ways to navigate the healthcare system.
Engage with family members and caregivers, ensuring they understand the care plan and their role in the patient's care.
Help plan and arrange for safe and timely discharges from hospitals or healthcare facilities, ensuring appropriate follow-up care is in place.
Job Requirements Education Qualifications Completion of Nursing Program Experience Previous clinical experience in healthcare settings is often preferred, especially in outpatient care or similar environments.
Previous lead or operations experience 2+ years of outpatient experience Staff scheduling experience Licenses and Certifications Basic Life Support (BLS) Certification Certifications in specific areas (e.g., IV therapy, wound care) may be beneficial and enhance employability.
