We are seeking on-call, base pay plus per-visit LVN Licensed Vocational Nurse to join our team in the Murietta/Temecula area!
You will be responsible for the weekend from 5pm on Friday until 8am on Monday morning.
Pay structure as follows:

Friday 5pm- Saturday 8am $100 flat fee for shift plus visit fee Saturday 8am- Sunday 8am $160 flat fee for shift plus visit fee Sunday 8am- Monday 8am $160 flat fee for shift plus visit fee

You will work with an RN who will triage calls and then visit patients if a visit is warranted after triage, delivering high-quality care to hospice patients.
Clinical experience is required and candidates with Hospice experience are highly desirable.

?Responsibilities:

Provide Skilled Nursing Care in a home or facility setting

Ensure the appropriate use of meds, DME, and supplies

Educate and support patients, families, and caregivers

Participate in IDT updates and updating the Plan of Care

Provide our patients and families the best care possible

?Qualifications:

Valid LVN License in California

One year or more of experience in either Home Health or Hospice is preferred, but not required

Previous experience in a hospice setting is a plus

Ability to build rapport with patients

Compassionate and caring demeanor

Excellent written and verbal communication skills
