Position Title: MDS/ Careplan Nurse

Reports To: Director of Nursing

PURPOSE:

Responsible for assessing and/or coordinating assessment of all residents through completion of the MDS in accordance with State and Federal regulations and facility policies.
Responsible for triggers and protocols associated with MDS for each resident.
Responsible to accurately determine resident level of functioning and procuring evidentiary clinical record documentation to support MDS scoring.
To assure that direct care personnel (i.e.
C.N.A.
's & floor nurses) accurately identify/document resident level of functioning within required time frames.
QUALIFICATIONS & ESSENTIAL REQUIREMENTS:

Current and unrestricted active R.N./L.P.N.
Illinois State License

Must be able to read, write, speak and understand the English language

Ability to coordinate a team effort in interdisciplinary care conference and Medicare meetings.
Ability to perform thorough and accurate assessments of all residents.
Ability to correctly complete the necessary documentation via the computer for the MDS in accordance with State and Federal regulations and facility policies.
Ability to maintain confidentiality

Must possess ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public

Must be knowledgeable of nursing and medical practices and procedures, as well as laws regulations and guidelines that pertain to long term care

Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
Must be physically capable of effectively hearing, seeing and speaking.
The duties of this position include, but are not limited to:

Coordinate in conjunction with the Care Plan Coordinator the completion of each resident's MDS in compliance with all State and Federal requirements.
Calculate triggers and develop resident assessment protocols for initiation of care plan.
Ensure that MDS's are completed in a timely manner and accurately identify each resident's level of functioning in accordance with RAI manual guidelines.
Ensure that each MDS accurately identifies the lowest level of functioning allowable to maximize reimbursement opportunities as allowable by Federal and State law.
Ensure that C.N.A.
's, nurses and IDT disciplines have accurately completed required clinical record documentation in order to correctly complete MDS scoring.
Ensure that this record completion is performed in a timely manner.
