The RN MDS Coordinator plays a key role in ensuring the accuracy of resident assessments and the optimization of care plans. Key responsibilities include: Leading and coordinating the development and completion of resident assessments (MDS) in compliance with current regulations and guidelines. Ensuring the MDS process is completed accurately and within required timeframes. Collaborating with the interdisciplinary team to create personalized care plans for residents. Continuously monitoring and documenting residents' progress and responses to care plans. Actively participating in the Quality Assurance and Performance Improvement (QAPI) process to enhance care quality. Communicating effectively with residents, families, and healthcare providers to ensure seamless care coordination. Staying informed on MDS regulations and guidelines to ensure compliance with industry standards. This role is essential in maintaining high standards of care and regulatory compliance within the facility.
