The House Calls Medical Assistant/CIS will fulfill a crucial role, supporting logistics, clinical care, and documentation in our innovative home-based care program.
The Home Visit Medical Assistant/CIS will work directly with a House Calls nurse practitioner on a daily basis, in collaboration with center-based care teams, to help identify and bridge gaps in care when patients return from the hospital or experience other challenges.Core functions:Preparing visit schedule, route, and logistics for all visitsAccompanying nurse practitioner on all house callsVital signs, medication reconciliation, patient / family educationDocumenting House Call note, managing referrals, orders and follow-upMaintaining supplies/equipmentOther duties as assignedWhat we're looking for:Genuine passion for primary care and reaching vulnerable patientsComfort navigating visits in patient homesWillingness to learn and be accountable for EMR documentation and workflowExcellent communication, follow-up, teamwork, and problem-solving skillsDesire to be a part of a fast-paced, innovative, quality-driven organizationRequirements:At least one year experience as a Medical Assistant + MA certificationMust pass all clinical competencies before supporting patient care, phlebotomy a plusDaily access to a vehicle with valid license and insuranceFlexibility to travel around ChicagolandBilingual (English and Spanish) a plusUS work authorization<
