Responsibilities:

Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care in a skilled nursing facility

Implement an individualized plan of care for patients' goals and provide clear communication with patients, clinicians, facility leaders and staff regarding care and services for enrollees

Work alongside pharmacists on medication management

Complete acute, follow up, discharge, and wellness visits as necessary

Make recommendations on transitional care, DME, home health services, or outpatient services after short stay

Timely response in chart completions and pharmacy recommendations

Provide oversight to TeamHealth clinicians providing coverage in the same facility

Experience as a medical director in a skilled nursing facility preferred. However, clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), geriatric, primary, emergency (EM), internal (IM), adult, post-acute, home health, hospice, palliative, and correctional medicine (HM).

Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated range of $50,000 to $77,000 annually with no cap on productivity income potential.
