The Medical Director for Population Health will be responsible for co-leading, with Directors and Vice Presidents from the Population Health and Central Services Departments, a range of clinical and strategic initiatives. The Population Health program is led by Oak Street's Chief Population Health and Chief Clinical Officers and focuses on the design and implementation of the Oak Street Care Model. The Oak Street Product team, which designs, builds and implements software tools to support care team workflows and meet their information needs, is a part of the Population Health team and therefore technology work will be part of the scope of the team and position. The Medical Director - Population Health will provide clinical leadership to a portfolio of programs and planning initiatives within this domain. Examples of potential projects include:
Centralized Telehealth ServicesClinical Teams within our call center, referrals and medical records departmentsLiaison with external contracted disease management companiesSpecialty Services embedded in the primary care centersPharmacy and Medication Management activitiesCore Responsibilities: In addition to clinical duties caring for Oak Street patients as a primary care provider 2.5-3 days per week in one of our Centers, the Medical Director is responsible for the following leadership and administrative duties:
Population Health program clinical leadership: This physician or Advanced Practice Provider will be the clinical leader for a set of programs within the Population Health and Centralized Services Teams. Most often, the Medical Director will be partnered with a dyad partner focused on operations for each program. As the clinical leader for the programs, it will be the Medical Director's responsibility to assure the clinical services are high quality, patient-centered and address Oak Street's broader goals. Depending on the program design, the Medical Director may be expected to provide direct clinical supervision to RN's, NP's, physicians, and/or other clinical or support staff involved in delivering care through these programs.Liaison to other clinical and operational leaders: Patients receiving care through these clinical programs will, in most cases, also be receiving primary care through Oak Street's network of primary care centers. Therefore, it is critical that all programs continuously communicate with the patient's primary care team members to assure patient-centered care. The Medical Director will be expected to consistently seek input from Medical Directors and other clinical leaders supporting and working within our primary care centers. The Medical Director will also be responsible for informing all stakeholders regarding program activities and efforts to assure excellent communication and collaboration.Organizational Strategy and Collaboration: The Medical Director for Population Health will participate in organization-wide planning and strategy including helping vet new opportunities, plan staffing, and hiring staff. The Medical Director will also collaborate closely with Population Health, Central Services, and Divisional/Regional leaders in these efforts. Additionally, the Medical Director may be asked to participate or lead workgroups or committees.Drive Culture: At Oak Street, we foster a culture of innovation and open dialogue, and we work with urgency. US work authorizationPrevious experience in a leadership position.M.D. / D.O. / APN / PA clinical training including an active license to practice.Physicians should be Board certified in Internal Medicine, Family Medicine, Preventative Medicine or Geriatrics.Fellowship training, professional degrees (e.g., M.B.A., J.D., M.P.H.), and/or 2-5 years of management consulting experience.Training and experience providing primary care is preferred.Prior clinical leadership or supervision experience is preferred.Experience with managed care and/or value-based practice and familiarity with payer-provider collaboration.Experience using a metrics-driven approach to analyze cost, quality, and satisfaction data to drive clinical strategy and program redesign.Experience developing and implementing new processes.Understand the basics of managed care.Ability to collaborate effectively with staff, providers, and a diverse group of leaders.Someone who embodies being 'OAKY'.What does being 'Oaky' look like?
