The Registered Professional Nurse assesses, coordinates, plans, directs, implements and manages the needs of
assigned patients throughout the episode of care with a focus on progress toward discharge, including during
transitions within the acute care stay. coordination of patient care provided by the caregiving team. patient care is uniformly provided to customers including referring facilities, hospital staff and patients in the
inpatient, ambulatory, and community settings.

Actively participates in performance and quality improvement activities and works toward enhancing customer/patient satisfaction. Initiates open communication, conveys positive intent, offers assistance. Demonstrates respect for individual differences of each person by acknowledging the essence of each person, appreciating and responding to unique, spiritual, personal and cultural backgrounds of patients, families and colleagues. Collects pertinent data relative to the patient's health or situation. Integrates knowledge of evidence-based assessment parameters (within defined limits) in care area. Prioritizes objective and subjective assessment data. Diagnosis and Planning 1. Analyzes assessment data to identify patient problems, formulate expected outcomes, and determine strategies for meeting the patient's individualized needs. Applies knowledge of disease processes to formulate appropriate nursing diagnoses. Documents problems, outcomes, and planning of interventions using standardized terms and technology. Collaborates with the patient, family, and interprofessional team to formulate an individualized plan of care. Demonstrates time management by prioritizing care to meet patient and family needs across the continuum. Implementation of the Nursing Plan of Care 1. Implements strategies to address health promotion, prevention of illness/injury, facilitation of healing, alleviation of pain/suffering, and supportive care. Performs/delegates interventions according to the Nurse Practice Act and AMC policy/procedure. Works to develop technical skills, safe patient handling, and infection control techniques. Coordinates care with the patient, family, interprofessional team, and community-based resources to affect safe transitions in accordance with federal and state regulations, utilizing the chain of command as necessary. Delivers patient/family education with respect to culture, values, developmental level, and readiness for learning. Uses ongoing assessment data to revise diagnoses, outcomes, plan, and implementation strategies. Re-assesses structures, processes, and timelines prescribed in the plan. Documents significant cues of patient improvement or deterioration based on the goals of the plan of care. Collaborates with consumers of healthcare and other caregivers in the conduct of care. Serves as a charge nurse for the unit/department, leading other caregivers within the professional practice setting. Seeks knowledge and competence that reflects current nursing practice and promotes future thinking. Contributes to quality nursing practice through participation in unit/hospital committees and shared governance. Serves as a preceptor and competency reviewer, evaluating one's own and others' nursing and patient care practice. Utilizes appropriate resources to plan, provide, and sustain evidence-based nursing services that are safe, effective and fiscally responsible. Organizational Requirements: ?
