Navajo Preference Employment Act

In accordance with Navajo Nation and federal law, TCRHCC has implemented an Affirmative Action Plan pursuant to the Navajo Preference in Employment Act. Position SummaryThe Medication Safety and Quality Officer is responsible for creating and ensuring staff attend and adhere to medication quality and safety programs, identifying opportunities to improve the safety of medication-use systems, and developing error-prevention strategies and process improvements targeted to reduce patient safety risks. Collaborates with health care providers having medication-related responsibilities to monitor and measure medication-use systems and implement error-prevention strategies with consideration of the aging processes and human developmental stages. Responsible for external and inter-department coordination and integration of pharmacy related information systems technology and solution development. Participates in the development and implementation of Pharmacy Services goals and objectives, policies, procedures, and budgets.
QualificationsNecessary QualificationsEducation:Doctor of Pharmacy (PharmD) from a school recognized by the American Council of Pharmaceutical Education.
License/Certification:Must have a current valid licensed to practice Pharmacy in a State, Territory of the United States, or the District of Columbia and maintained throughout employment in good standing.Must have and maintain Basic Life Support (BLS) Certification with American Heart Association (AHA) throughout employment.Experience:Must have at least three (3) years of clinical pharmacy experience, specifically including specialized Quality Improvement Coordinator activities.Must have knowledge of medication-use safe ISMP practices; initiatives; National Patient Safety Goals.Other Skills and AbilitiesA record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. A successful interview exhibiting leadership skills and knowledge of laboratory functions. All employment references must address and indicate success in each one of the following areas:

Physical:This position typically is a sedentary position. Therefore, this position requires the incumbent to be able to sit, stand, walk, and maintain balance frequently (over ½ the typical workday). The incumbent will occasionally need to be able to drive, bend, climb, kneel, crouch, twist, and reach. The incumbent must have ability to lift up to 24 pounds and pushing and pulling up to 34 lbs. occasionally (less than ¼ of the day rarely). Incumbent must be able to be lifting, pushing and pulling up to 10 pounds frequently. This position requires the incumbent to have the sensory abilities for far vision, near vision, color vision, depth perception, seeing fine details, hearing normal speech, hearing overhead pages and telephone use for prolonged (3/4 workday) use during a typical workday. Incumbent hand manipulation ability with frequent simple and firm grasping and fine manipulation. Requires prolonged use of a keyboard.
Mental:The incumbent identifies deficiencies in program operations and pharmacy services and makes recommendations for change in procedures and/or guidelines to ensure the delivery of quality patient care services. The incumbent must be able to cope with high level of stress, make decisions under high pressure, cope with anger/fear/hostility of others in a calm way, concentrate, handle a high degree of flexibility, handle multiple priorities in stressful situation, work alone, demonstrate high degree of patience, adapt to shift work, and work in areas that are close and crowded frequently (more than ½ day). Incumbent may occasionally (less than ¼ work time) need to manage altercation. This position requires the incumbent to be able to accept a flexible schedule to meet unit needs.
Environmental:This position typically will work in an environmentally control area. The incumbent occasionally (less than ¼ time working) be exposed to infectious disease, chemical agents, dust, fumes, gases, extremes in temperature or humidity, hazardous or moving equipment, unprotected heights and loud noises.
ResponsibilitiesEssential FunctionsServe as the authoritative resource on medication safety and serve as Chair of Medication Safety Committee.Participate in the development, implementation, and integration of patient-focused medication quality and safety initiatives in collaboration with other leaders and key constituents.Participate in, or lead, multidisciplinary teams concerned with medication errors, adverse drug events and reactions, near misses, policy review, safe medication use, new product review, and patient safety to identify risk points and prioritize system improvements to reduce the potential for medication errors and patient harm.Collect, review and analyze established departmental and system-wide Medication Quality and Safety programs including medication-use, medication error, adverse drug reactions, and continuous quality improvement data. Identify needed improvement; develop and lead implementation of high leverage error-reduction strategies.Ensure compliance state and federal medication safety regulatory and legal requirements.Investigate medication safety events or issues, including specific error reports. Utilize data-driven performance-improvement methodologies and tools to develop recommendations for action.Encourage system-wide medication error reporting. Foster a culture of safety through "lessons learned" education and communication across the entire system. Support principles of high reliability organizations (HRO) within medication-use processes and workflows.Provide medication safety education and presentations on safe medication use to diverse audiences (Pharmacists and technicians, nurses, respiratory therapists, medical students and physicians).Manage medication safety issues caused by drug shortages, replacement drug products, and Look-alike Sound-alike drugs.Maintain knowledge of trends and developments in the patient safety field. Collaborate with external medication safety organizations, including Institute for Safe Medication Practices (ISMP) and the Food and Drug Administration (FDA) Medwatch program. Share locally identified and reported medication quality and safety issues with national organizations and manufacturers as appropriate.Provide guidance and coordination for medication related information systems and technologies.Provide operational, clinical, and safety recommendations related to installation, maintenance and enhancements of Pharmacy related systems and technologies. Ensure information system and technology processes support established clinical standards of care, meet, or exceed patient care quality standards, are in compliance with various regulatory agencies, and integrate with other departments.Participate in the development, implementation, and evaluation of pharmacy information systems and technology to consistently meet or exceed customer's needs, through effective continuous quality improvement practices.Maintains knowledge of leading-edge developments and best practices by participation in ongoing independent study, education-related professional activities, and affiliations.Functions as a work leader for all Pharmacy Performance Improvement work teams, guiding the team(s) through a variety of steps including cause and effect diagrams, prioritization of items, and the Plan-Do-Check-Act (PDCA) method to address issues or problems. Serves as a resource to other staff for all performance improvement questions, requirements, and activities. Serves as a liaison for the Pharmacy PI program, interacts with other departments, and serves on facility-wide Pl teams as required. Assures the Pharmacy PI program is consistent with current ASHP guidelines and meets all the standards as set forth by The Joint Commission.Serves as the expert on pharmaceuticals, pharmacokinetics, and pharmacotherapeutics. Using an extensive and intensive knowledge of the drugs and other pharmaceuticals available, their nature, action, and interaction, provides authoritative instructions, advice, and assistance to prescribing and dispensing officials on effectiveness, suitability, hazards, and other aspects of their use. As subject matter expert to the Therapeutic Agents Board, recommends which items will be stocked based on their effectiveness, economy, and appropriateness for the intended use. Serves on a variety of committees, boards, and other bodies representing the pharmacy specialty and providing authoritative input on pharmacy related matters.Monitors the dispensing of all pharmaceuticals in the facility. Analyzes patterns and trends to determine appropriate, effective, and economical prescribing and dispensing of drugs. Where improvement in these areas can be identified, counsels or recommends alternatives that will still satisfy the medical needs of the situation. Looks for indications of misuse or abuse of pharmaceuticals and recommends corrective or remedial action. Inspects pharmacy operations in the main and any remote facilities to ensure compliance with operation instructions and quality assurance criteria and procedures. As tasked, provides pharmacy input to performance improvement reviews and similar processes.Is privileged by the medical staff and administration to develop, with the attending physician, pharmacotherapeutic regimens for especially difficult or complex cases. Of particular concern are cases involving multiple serious or life-threatening conditions with conflicting drug requirements, serious drug reactions or interactions, or other factors not receptive to standard or more common specialized approaches. Monitors progress and response to the regimen, modifying dosages or methods of administering as necessary. In emergency situations may change medication or significantly alter regimen without prior approval of physician but will be subject to mandatory post action review.Participates in, conducts, or approves studies involving pharmaceuticals and pharmacy related issues. Designs and conducts studies intended to evaluate particular drugs, their effectiveness and suitability, or to study the reaction of diseases, patients, and conditions to drug-based treatment. Reviews and approves pharmacy aspects of all protocols and investigative studies. Closely monitors and controls pharmacy aspects of all experimental and investigative drug use. Prepares related reports and documentation.Prepares and conducts or oversees training of physicians, pharmacists, and other health care providers on the use of drugs and other pharmaceuticals. Serves as pharmacy residential coordinator in accordance with university standards and recommendations. Monitors training of residents, interns, and follows to ensure drug related issues are fully and correctly addressed within their curriculum and that the required levels of skill have been attained. Recommends or initiates changes and updates instructions, regulations and other guidance as needed.Ensure proper PPE is worn at all times while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield.Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates).Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction.Performs other duties as assigned.
