Job Summary This paragraph summarizes the general nature, level and purpose of the job. The Mental Health Resource Coordinator will serve as an advocate and point of contact for patients and families receiving treatment in the Child and Adolescent Psychiatry Division including the Crisis Program.  The MHRC will assist with navigating medical systems and understand available resources in LPCH and the community.  The MHRC has the goal to identify barriers to medical treatment, ensure access to appropriate resources, improve patient experience, and resolve issues that impacts care progression.  The MHRC will perform delegated functions of the case manager, social worker, and interdisciplinary team. She/he will actively communicate with external agencies to create a smooth transition of care. The focus will be on understanding the needs of the patient and facilitating the implementation of the recommendations of the treatment team. Provide coordination for individuals in Crisis Program to access resources in the community to improve patient outcome and experience. Provide coordination of care for children and adolescents who are receiving care in the Child and Adolescent Psychiatry Division Functions as a resource for the individual's family, assist in understanding care team's referral and the steps needed to be taken next. Develop an individual plan for coordination with consideration for provider teams, individual and family. Evaluate patient and family situation and coordinate with provider team Identify barriers to crisis treatment and facilitate resolution of barriers with appropriate care team. Coordinate care with relevant providers at Stanford.  When appropriate referral coordination with care team's approved community resources.       Participate in division and clinic meetings to improve quality of care in the Division Represent LPCH child psychiatry Program goals and assist in developing the program's efficiencies. Minimum Qualifications Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying. Education : Bachelor's degree in a work-related discipline/field from an accredited college or university Experience : Four (4) years of progressively responsible and directly related work experience License/Certification : None Knowledge, Skills, & Abilities These are the observable and measurable attributes and skills required to perform successfully the essential functions of the job and are generally demonstrated through qualifying experience, education, or licensure/certification.  Knowledge of Medical Record Documentation. Ability to assist Care Team in assessing and addressing physical, psychological, social and resource needs of the patient and family/caregiver and provide appropriate support as needed. Ability to assess patients displaying severe challenging behaviors Ability to integrate patient care with other members of the interdisciplinary health care team in assigned area of responsibility to ensure quality care. Assist patients and their parents in acquiring academic, social, and adaptive life skills Knowledge of computer systems and software used in functional area. Ability to work with multiple cases. Ability to interact in a respectful, professional manner in accordance with hospital policies and procedures. Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data. Typically, new team members join at the minimum to mid salary range.
