Employment Type: Full time Shift: Day Shift Description: Facilitates multidisciplinary comprehensive patient care by coordinating multidisciplinary patient care team activities. This includes patient entry into the program and coordination of care through diagnosis and treatment. Keeps appropriate physicians aware of progress and facilitates care for the patient. Educates staff and community concerning specific patient population. Monitors service, quality, and financial parameters with the manager for the program and reports to administration and executive teams. Recognizes and consistently applies knowledge of Loyola's 14 Forces of Magnetism. Coordinates care in order to facilitate comprehensive and individualized care to patients/families. Coordinates and collaborates with the multidisciplinary staff to develop patient care that is consistent with the overall standards of care and mission statement of Loyola University Medial Center. Develops and coordinates systems which ensure the screening, early diagnosis and treatment of specific patient populations. Coordinates and collaborates with the multidisciplinary care team members and other health care providers to promote continuity of care, with emphasis on the cancer patients. Assumes the responsibilities of other coordinators in their absence. Assesses and triages patients for timely access, appointments, and appropriate care. Oversees and coordinates the clinic flow including follow through on recommended treatment plans. Tracks cancer patient's flow through work-up, surgery, radiation and chemotherapy as appropriate for the population while appraising physicians of the patient's status. Participates as a clinical resource for staff, management, and the other health care professionals. Collaborates with Can Help, Patient Financial Services, and clinical areas to facilitate access into the Multidisciplinary clinic. Coordinates the referral management process for the clinic patients. Oversees an exit interview process with each patient to ensure patient understanding of treatment and follow-up. Identifies patient/family and staff learning needs within the context of the Multidisciplinary Clink and strives to meet those needs. Develops, conducts and participates in formal in-service programs related to disease, screening, diagnosis, treatment, and care for specific populations. Assists in the development of individual or group teaching programs for patients and families based on their identified needs. Serves as a preceptor/role model for personnel, students, and other health care professionals. Collaborates with and participates in community outreach programs. Collaborates with primary care physicians to enhance referrals to the Multidisciplinary Clinic. Attends and participates in appropriated continuing education, staff development programs, conferences and grand rounds as possible. Participates in professional organizations. Reviews current professional literature and participates in professional activities. Assumes responsibility for own development in all aspects of the coordinator role. Utilizes institutional resources to enhance professional growth. Collects data on quality indicators and tracks over time. Works with Data Management in identifying patients for research protocols. III. Position Requirements : Minimum Education: Required: Associates Degree Preferred: Bachelors Degree Specify Degree(s): Nursing. Completion of BSN within five years of employment is required.
