We are searching for full time NP's who are bilingual, with verbal and written fluency in Spanish. The Telehealth Nurse Practitioner in Dementia Care is an essential member of our remote healthcare team, dedicated to providing specialized care for patients with dementia and their caregivers. This role focuses on delivering comprehensive, interdisciplinary care through telehealth platforms to ensure accessible and effective treatment. Travel Requirements: This position is fully remote, and we provide the necessary technology to work from home. Occasional travel to our headquarters in Chicago, IL and/or other locations may be expected. Key Responsibilities: Provide specialized, remote care and support to dementia patients and their caregivers. Assess and address the psychosocial needs of patients, families, conservators, and caregivers in a culturally diverse environment. Utilize telehealth technology to conduct assessments, monitor progress, and adjust treatment plans. Develop and implement individualized care plans in collaboration with the clinical team.
