The Columbus Organization's Support Coordination Services in NJ continue to grow with an immediate need for an experienced Support Coordinator in Mount Holly, NJ. We are looking for a motivated, organized, energetic, technologically proficient individual who is not only good at writing and paperwork but also great with individuals with disabilities. We seek someone who shares our passion for working with adults with disabilities specifically.
Support Coordinator Responsibilities:Manage services for individuals with disabilities.Conduct intake meetings.Develop a plan for services based on the needs of the individual.Facilitate the start of services for the individual with state vendors.Monitor the individual and the plan on a monthly basis.On call for emergencies.By joining the Columbus Team, you will be a valued and appreciated team member that will enjoy the following benefits:
