Nurse Case Manager

The Nurse Case Manager will provide help to high-risk infants and children getting health care and developmental services.
The Nurse case manager will complete the following:


promote optimal growth and development of infants. provide emotional support and relevant referrals to families for essential services. work with the mother to reduce stress and help explain identified medical conditions for further evaluation, diagnosis and treatment. responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from High-Risk Infant follow Up Program from the State of Illinois. Requirements

Requirements Bachelor's degree in nursing from an Accredited College or University Minimum of 2 years' experience working in Community Health or Maternal Child and Health nursing Active nurse License in the state of Illinois PHYSICAL DEMANDS:
