Position Title: Nurse Liaison, Pharmacy Home Infusion Services
Job Summary:
Serve as an expert clinical consultant to hospital staff, clinical educator, and advocate for patients, with the goals of raising awareness about the benefits of home infusion and optimizing the transition of care for patients referred to home infusion.
As a clinical expert, you will educate patients, physicians, case managers, and all customers regarding disease states, infusion therapy, transitional care management, and other Home Infusion programs and services. You will facilitate the timely placement of patients requiring home infusion/infusion suite or enteral services by gathering preadmission data and collaborating with the hospital discharge team or outpatient clinic care team, including initial patient set-up/training and ongoing patient/caregiver education. You will spend some of your responsibilities interfacing with healthcare professionals and hospital staff to strengthen partnerships and grow business through referrals.
Candidates are nurses with excellent clinical, interpersonal and communication skills. Must work independently and effectively manage tasks in a fast-paced environment. Must be a team player with a positive attitude who works well in a cross-functional, matrixed environment.
Basic Education, Training, and Experience Required: Education:
Required: Graduate of an accredited school of nursing required
Preferred: Bachelor's degree from an accredited college or university in marketing, communications, health care or a related field
Credentials:
Required: Current Registered Nurse License in any state or an RN Required Multi-state Nursing License from a participating state in the NLC (Nurse Licensing Compact). CPR Certification.
Experience:
Required: Minimum of three (3) years clinical (RN) nursing experience. Basic level skill in Office Suite (Word, Excel, PowerPoint).
Preferred: Healthcare Sales/marketing experience. Minimum three years working in a health care or pharmacy organization. Experience with Home Infusion or Home Health. Intermediate level skill in Office Suite (Word, Excel, PowerPoint)
Special Skills:
- Thorough knowledge of computer systems and the ability to effectively utilize software applications (specifically Microsoft Word®, Excel® and Power Point®) to support Home Infusion services.
- Good problem solver who defines, clarifies and sets priorities for producing quality results within established timeframes.

Key Accountabilities:

- Interface with healthcare professionals and hospital staff to strengthen partnerships and grow business through referrals.
- Provides a positive and effective representation of Home Infusion and Pharmacy
Services and introduces the full scope of services offered to hospital case managers, physicians, nursing agencies and other referral sources.
- Works closely with the Home Infusion Manager to market and grow Home Infusion Services and maintains positive referral source relationships through responsive customer service with referral sources and patients.
- Collaborates with referral sources to facilitate care coordination for new patients and provide clinical support.
