Job Description
The Nurse Navigator plans, coordinates and delivers health services to patients receiving care within the defined Service Line.  This includes working collaboratively with other health providers to assure coordinated care throughout the service line's continuum of care.  The nurse navigator will assist with the coordination of services and will provide support and education throughout diagnosis and treatment for patients.  She / he will serve as a professional resource in the areas of the Service Line.

Act as a liaison between hospital floor staff and clinic staff to coordinate outpatient visits for transitioning patients.
Serve as a consistent contact for patient throughout the treatment, providing follow-up calls to ensure all testing and orders are being followed/completed.
Work with providers and staff on outbound referrals.
Develop, coordinate, and continually improve processes pertaining to the Service Line coordination of care.
Serve as a resource and consultant for the Service Line as well as the community, developing a presence in the professional and general public. Facilitates referrals by collaborating with referring providers
Qualifications Education:

BSN or Master's Degree

Experience:

5 years nursing experience
Experience within the Service Line.  Experience in a multi-specialty environment. Experience with home care/wound care ideal

Licensure:


Licensed as a professional RN in the state of Connecticut
