In collaboration with the multidisciplinary team, the Spine Nurse Navigator is responsible for supporting the patient throughout the continuum of care, functioning as a patient advocate, liaison, and educator.
Key areas of responsibility
· Performs daily patient rounds during inpatient hospital stay; reinforces patient education regarding disease process and rehabilitation goals. This includes coverage for our Elective, Urgent, and Emergent Spine populations.
· Collaborates with the Ortho Trauma Nurse Navigator to support the Trauma Spine population throughout peri-operative and post-operative touchpoints.
· Serves as a liaison for the Spine patient population between the patient and surgeon, inpatient teams and appropriate outpatient services as determined by patient's clinical needs.
· Performs pre-operative and post transition outreach to identify problems and provide early intervention.
· Provides support to all Spine patients during any Readmissions to the hospital as well as provides post-transition outreach after a return to the Emergency Department.
· In collaboration with other healthcare team members, provides oversight and direction of patient continuous quality improvement metrics, such as: patient experience, falls, readmissions, percentage of SNF referrals, and other quality metrics. Participates in daily clinical progression rounds and weekly prospective patient rounds.
· Participates in clinical quality care improvement projects and Research, resulting in measured outcomes to optimize the patient experience.
· In partnership with Nursing Leadership, recruits and promotes participation in patient engagement committees
· Provides perioperative education to optimize patient outcomes
· Supports the preoperative and postoperative patient in a holistic manner by engaging appropriate resources to meet their needs.
· Participates in workshops, seminars and community outreach programs to support and advance the Bone and Joint mission.
· Participates in clinical research to support and enhance evidence-based nursing practice.
· Ensure proper Patient Reported Outcomes (PRO) captures on selected virtual platforms.Qualifications Education
· Bachelor of Science in Nursing
· Masters (Preferred)
Experience
· Minimum 5 years clinical nursing experience preferred
· Service Line and/or operational experience (Preferred)
Licensure, Certification, Registration
· Active CT Registered Nurse license

· Demonstrated management skills reflecting self-motivation, initiative and fiscal responsibility.
· Skills and knowledge of Microsoft office tools (Word, Powerpoint, Excel).
