This individual will be accountable for reducing the re-admission rate for a defined high-risk population in the Northwest Region. This accountability will include providing clinical services in the home and clinic settings which effectively reduce preventable re-admissions in the aforementioned population. This includes providing direct patient care, as well as triaging to appropriate coordination, social and behavioral health resources. The role includes the NP taking full-responsibility for the patient from post-discharge to sustainable stabilization—not at high-risk for subsequent re-admission. There will be a strong focus on co-morbid CHF/COPD patients.
Essential Duties and Responsibilities include the following: Provides care to defined high-risk members, either in the home or at the clinic.Adheres to evidenced based clinic guidelines.Appropriately documents, per Regal and regulatory compliance rules, all care provisioned.Ensures appropriate follow-up for patient including social and behavioral services.Tracks clinical/social issues, patients seen.All visits must be conducted timely, and adhere to group/health plan guidelines and protocols.All other duties as assigned.The pay range for this position at commencement of employment is expected to be between $130,000 - $140,000 annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements Education and/or Experience: Current Certification as Nurse Practitioner.Experience with COPD/CHF specific care.Experience with high-risk, complex patients.Experience delivering care in home-settings preferred but not required.Minimum 3-4 years experience in health care.Excellent verbal and written communication skills in English.Proficient at Microsoft Office products.Ability to work independently in the field, while still meeting team deadlines and requirements.Bi-lingual (Spanish) preferred but not required.
