Responsibilities:
Perform thorough medical assessments with attention to detail.
Oversee the operations of a primary care facility, catering specifically to Medicare and Medicaid patients.
Skillfully diagnose and manage both acute and chronic illnesses.
Create and execute individualized patient care plans.
Offer invaluable health coaching and educational support to patients and their families.
Foster seamless collaboration with fellow healthcare professionals to ensure comprehensive patient care.
Maintain accurate documentation and coding utilizing electronic medical records systems.
Requirements:
Possession of a valid Nurse Practitioner license in [state].
Completion of a Master's degree program in Nursing.
Demonstrated experience of at least 2 years in a Nurse Practitioner role.
Possession of an NPI for independent billing.
