Tapestry Health is seeking an on-site Nurse Practitioner or Physician Assistant to join our team providing medical care to patients in a Skilled Nursing Facility in Pikeville, Kentucky.
This is a full-time position, and the hours are Monday-Friday, 40 hours per week.
No weekend, nights, call or holiday work.
As a member of our group, you will have the freedom to work independently with the knowledge that your team members are always just a call away.
PRIMARY RESPONSIBILITIES: Be available to care for patients in the assigned facilities at the scheduled times. Work with the facility to establish the most efficient workflow for you and them. Assist with efforts to prevent hospitalization of patients by proactively seeing and treating patients for change of condition. Play an active role working with the attending in monitoring medication use. Work closely with the facility in managing patients with weight loss, pain, wound care and use of antibiotics and IVs. Conduct full assessment of patients to support quality care. Communicate with the patient's family as an essential part of your first visit with the patient. Conduct monthly calls to the family to discuss the patient's status as necessary for individual patients. Address advanced care directives when appropriate. Work in tandem as a member of the TapestryHealth clinical team with other designated specialty Clinicians and Consulting Physicians. Complete all notes within 48 hours of seeing the patient.
MINIMUM REQUIREMENTS Licensed in Kentucky Experience with geriatric population required Strong assessment and communication skills Knowledge in medication management and de-escalation Must provide proof of the COVID vaccine Experience with providing care in ICU, ER, Urgent Care, Primary Care, and Surgical will be considered
