Position Summary: We are seeking a dedicated and experienced Nurse Practitioner to join our team. The ideal candidate will be passionate about providing high-quality, patient-centered care and will have a strong background in wellness, preventive care, and chronic disease management. The Nurse Practitioner will collaborate with our team of healthcare professionals to deliver comprehensive care and support to our patients.
Key Responsibilities: Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests.Develop and implement individualized care plans based on patient needs and wellness goals.Provide preventive care services, including health screenings, vaccinations, and lifestyle counseling.Manage chronic conditions such as diabetes, hypertension, and obesity through personalized treatment plans.Prescribe medications and treatments as necessary, following established protocols and guidelines.Educate patients on health maintenance, disease prevention, and wellness strategies.Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.Maintain accurate and up-to-date patient records in compliance with HIPAA regulations.Participate in continuing education and professional development activities to stay current with best practices and advancements in the field.Qualifications: Master's or Doctorate degree in Nursing from an accredited program.Current Nurse Practitioner certification and licensure in the state of Texas.Minimum of 2 years of experience as a Nurse Practitioner, preferably in a wellness or preventive care setting.Strong clinical skills with the ability to perform comprehensive patient assessments.Excellent communication and interpersonal skills, with a patient-centered approach to care.Ability to work independently as well as part of a multidisciplinary team.Proficiency in electronic health record (EHR) systems.Commitment to ongoing professional development and education.Home Office Requirements: HIPAA-compliant office setup.Equal Employment Opportunity (EEO) and Affirmative Action CommitmentWays2Well is committed to fostering an inclusive environment that values diversity and provides equal employment opportunities to all.
