Position Summary:We have an exciting opportunity to join our team as a Nurse Practitioner - Part Time -Days. In this role, the successful candidate Nurse practitioners (NP) are dedicated to providing quality, professional, cost-effective health care to all patients at Long Island Community Hospital.At LICH, NPs provide services as a licensed independent practitioner (LIP) collaborating with a licensed physician fulfilling a broad range of medical services that would otherwise be provided by the physician. A NP shall be permitted to provide any service within his/her scope of training/skills outlined in the collaborative agreement between the NP and the collaborating physician that has been approved by the Credentials Committee. NPs may provide care for all patients throughout the hospital as permitted by their certification and licensure. NPs work collaboratively with the patient's physicians, assisting in providing high-quality care, and a well-planned, timely discharge. NPs provide patient-centered care as members of interdisciplinary teams emphasizing evidence-based practice and accepted standards of care. A NP shall document according to the practices included in the Clinical Documentation Initiative (CDI). NPs with Registered Nurse First Assistant (RNFA) documentation working in surgery have the primary responsibility of assisting in surgery. A NP working in surgery must have a collaborative agreement with a surgeon. A NP working in surgery will also perform surgical functions in other areas of the hospital as needed by the surgeons or the Chief PA. A NP on the Trauma Team must have privileges to first assist in surgery, experience in emergency medicine, and current Advanced Trauma Life Support (ATLS) documentation, or acquire it within six months. A NP on the Trauma Team may be required to be on-call.
Job Responsibilities:
Assessment and Reassessment of Patients


Introduces/identifies self as a nurse practitioner
Assists the physician with patient management focusing on quality care and a well-planned, timely discharge
Communicates professionally and effectively with the patient's physician, and as appropriate, consulting physicians, case managers, nurses, and other Hospital staff any patient information that may affect discharge or the patient's hospital course.
Performs initial and ongoing assessment/history and physical examination
Assesses/reassesses patient's condition and care plan
Orders and interprets tests/results(lab, radiology, ekg, etc.) based on patient need
Orders medications and treatments
Documents care in the chart in accordance with hospital policies and procedures
Formulates diagnoses
Formulates and implements patient care and treatment plans. Discusses patient care and treatment plans, expectations, needs, and any other information with physicians, nurses, case managers, and other hospital staff. Uses established Long Island Health Network (LIHN) guidelines, if applicable.
Obtains consents for blood/blood product transfusion, CAT scan and MRI contrast administration, and for procedures performed by the PA
Initiates consults with physicians and other hospital staff including those for hospice and palliative care
Initiates and participates in care related to end-of-life issues including Do Not Resuscitate/Intubate (DNR DNI) and Medical Orders for Life Sustaining Treatment (MOLST) documentation
Manages medical/surgical patient care including emergencies requiring BLS/ACLS/PALS (if adult or family NP)
Performs Trauma assessments/reassessments following ATLS (Trauma Team)
Performs peri- and intraoperative assessments/reassessments
Performs peri- and intraprocedure assessments/reassessments
Rounds with physicians and other hospital staff
Orders, applies, reapplies, and removes restraints
Documents in patient chart or the computer all pertinent patient information following departmental and hospital policies, and using the CDI education.
Writes legibly
Dates, times, and signs all chart entries
Uses name stamp
Documents any patient or family education
Documents results of tests ordered, or disposition of care
Completes all procedure notes
Documents communicationwith physicians
Signs, dates, times verbal/telephone orders by the end of each shift
Fully completes and submits billing forms by the end of each shift
Completes discharge documentation including discharge/transfer summaries, discharge instructions, writing/issuing discharge orders, prescriptions, and any other associated documentation required for discharge


Technical Procedures


Explains procedure to patient/family
Uses sterile technique, including barrier precautions, when needed
Performs procedures such as IV insertion, foley catheter insertion, naso/oro gastric tube insertion, central venous line removal, etc.
Sutures/ provides wound care
Splints
Assists in surgery
Assists with cardiac catheterizations
Performs CAT scan/MRI injections
Performs other radiological injections such as nuclear studies
Performs invasive procedures such as PICC insertion, central venous line insertion, chest tube insertion
Performs surgical procedures such as chest tube removal, drain removal, etc.
Assists radiologist with procedures such as biopsies, drainage procedures, special procedures, etc.
Performs radiological procedures such as paracentesis, thoracentesis, etc.

Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $67.27 - $89.61 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
