Tapestry Health is seeking an on-site Nurse Practitioner or Physician Assistant to join our team full-time providing medical care to patients in a Skilled Nursing Facility located in Florissant, Missouri.

The Nurse Practitioner will diagnose and treat health conditions and work collaboratively with the facility medical staff.PRIMARY RESPONSIBILITIES: Conduct comprehensive health assessments of residents, including physical examinations, medical histories, and medication reconciliation.
Develop and implement individualized care plans in collaboration with the interdisciplinary team, considering the unique needs and preferences of each resident.
Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to aid in diagnosis and treatment planning.
Prescribe medications and treatments within the scope of practice and in accordance with state regulations and facility policies.
Participate in care initiation for our chronic care management services.
Provide education and counseling to residents and their families regarding health maintenance, disease prevention, and self-care strategies.
Collaborate with physicians, nurses, therapists, social workers, and other healthcare professionals to ensure coordinated and holistic care delivery.
Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall quality of care and resident outcomes.
Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system within 72 hours of seeing patient.
All other duties as assigned in support our products and services.MINIMUM REQUIREMENTS Current licensure as a Nurse Practitioner in Missouri Board certification required.
Experience with geriatric population preferred.
Experience with providing care in ICU, ER, Urgent Care, Primary Care, and Surgical will be considered.
Strong assessment, interpersonal and communication skills Ability to work independently and as part of a team.
Knowledge in medication management and de-escalation Proficiency in using electronic medical record systems and other healthcare technology tools.
Ability to travel occasionally to facilities within your region as needed to complete initiating patient visits.Salary & Benefits: In addition to a competitive base salary, you will also be eligible for incentive comp pay.
