As a Nurse Practitioner In-Home Assessment you will:Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visitEducate and counsel patient and family based on assessment results, including recommendationsPerforms follow up visits in transitional care QualificationsCurrent RN and NP licensure in the state of practiceCurrent CPR certificationMust be enrolled in both Medicare & Medicaid Posted Min Pay Rate USD $85.00/Yr.
