Our Palliative Care program provides specialized medical care for people with a serious illness. This type of care is focused on providing relief from the symptoms and stress of a serious illness. The goal is to improve the quality of life for both the patient and the family. We have an outstanding Palliative Care team and we are looking to add a Nurse Practitioner. SUMMARY This position will work in collaboration with the Palliative Care Physician, Director, and interdisciplinary team. The Nurse Practitioner provides clinical expertise, ensuring standards of care and providing consultation and expertise in the delivery of patient care. Activities include consultation as requested, provision of patient care, and clinical oversight. The role involves working with physicians and clinical providers in conducting patient visits, face-to-face encounters, and other patient management encounters.

JOB DUTIES AND RESPONSIBILITIES Patient Care: Works collaboratively with the Palliative Care Director and other team members to ensure smooth day-to-day operations within the teams, ensuring the care provided to patients meets agency and regulatory standards, quality patient care, and effective and efficient resource utilization. The Nurse Practitioner plays an integral part in the development of a patient's comprehensive care plan by identifying symptoms of suffering and managing patient and family-centered care holistically in establishing realistic goals of care. Provides clinical expertise and oversight in conjunction with other clinical providers. Conducts visits on existing and new patients under the supervision of the Physician. Ensures appropriate multidisciplinary communication, collaborative working relationships, and a customer-oriented environment responsible to the needs of external/internal customers. Participates in orientation of new staff, training of staff, and is a resource to team members. Provides physician backup and coverage responsibilities where appropriate. Prescribes medications (non-narcotic) to the extent delegated and licensed. Orders treatments and durable medical equipment as indicated. Consults with physician or designees as needed, collaborates with other physicians as needed. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice. Discharges patients as condition indicates and/or patient requests, completing discharge summaries. Complies with applicable laws and regulations with respect to Collaborative Agreements. Participates in the development of the Plan of Care on all patients in conjunction with the Interdisciplinary team. Re-evaluates the patient's physical, psychosocial, and spiritual needs and notifies the appropriate member of the Interdisciplinary Team. Refers to and consults with other disciplines as indicated by patients' and family needs and Plan of Care. Supports, understands, and educates patients and their families on the patient's diagnosis. Provides cost-effective care within the fiscal parameters allotted. Informs the physician and other members of the interdisciplinary team of changes in the patient's condition and needs. Counsels with patient and family regarding physical and emotional needs. Able to determine the appropriate level of care to meet the patient's needs. Maintains a professional relationship and uses good communication skills with nursing home staff. Provides education and mentors staff, when requested. Performs supervisory visits, when required. Administrative Activities: Assist Branch Office Manager with scheduling tasks. Provides AIM administrative coordinator availability at least two weeks in advance. The NP will be expected to see patients during their scheduled timeframe. Will follow all scheduling expectations documented in this section. Completes documentation for each visit, including assessment tools, and submits within 7 days of visit. Assures necessary referrals are placed based on assessment results (social worker, chaplain, NP visit, etc.). Attends IDT meetings to discuss care and future plans of patients on the service, as requested by Program Director. Complies with department and regulatory requirements for documentation. Accounts for activities performed and submits documentation for billing to be processed within the allotted timeframe. *This is a brief list of job duties.
