This position serves in an expanded nursing role working in collaboration with the division Family Medicine providers that provide care to prenatal and postpartum patients. This position will coordinate care and provide support to the providers to ensure a care model that is conducive to the delivery of quality, efficient, and cost-effective healthcare services. Primary focus is on women's health, pregnancy, postpartum care and care of newborn. Assist with the transitions from the hospital to home and follow-op of recommended post-delivery care for both mom and newborn.
