Top Client in need of Manager of Case Management Job Summary and Qualifications The
Manager for Case Management Services reports to the Case Management
Director and is responsible for promoting patient-centered care by
coordinating all aspects of hospital based case management for his/her
assigned area of responsibility in alignment with the goals of the Case
Management Department. The Manager for Case Management is accountable
for the overall day-to-day oversight and management of the case
management program including coordination, supervision, and
administrative oversight of the case management team. The Manager for
Case Management functions as an expert clinical practitioner, case
management subject-matter expert, resource, advisor and leader for the
members of the case management team. The Manager for Case Management
supervises and monitors professional and support staff and ensures that
effective care coordination and case management practices are consistent
with hospital policies, and applicable regulations and guidelines. The
Manager for Case Management monitors quality of the interactions,
documentation and assessments of the case management staff. The Manager
for Case Management coordinates activities that promote quality outcomes
and patient throughput while supporting a balance of optimal care and
appropriate resource utilization.
You will serve as a role model and functions as a clinical resource for staff. You will supervise and facilitate development of staff and oversees the day-to-day tasks/activities in the department. You will ensure staff are competent in the use of InterQual to progress the plan of care. You
will work collaboratively and maintain active communication with
physicians, nurses, and other members of the interdisciplinary care team
to effect timely, appropriate patient management. You
will monitor the quality of CM interactions with patients, families,
physicians, and the multidisciplinary team by regularly shadowing staff
to ensure clear understanding and adherence to department processes. You
will monitor quality of documentation and assessments by regularly
auditing the case managers' documentation. Participates in clinical
performance improvement activities. You
will communicates effectively with patients, family, medical staff,
care team members, students, supervisors, and outside contacts. You will assume responsibility of the department in the absence of the Director. You will assume responsibility for scheduling and staffing of department. You will assume responsibility for department education and competencies. You will complete daily, weekly and monthly reports as assigned by the Director of Case Management. You will cover staff assignments as needed. You will seek ways to control costs without compromising patient safety, quality of care, or the services delivered. You
will adhere to established policy and procedure and standards of care;
escalates issues promptly through the established chain of command. You will demonstrate knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives. You
will serve as an advocate for patient's rights, needs, and values;
ensures that patients' ethnic, cultural, or religious values, beliefs,
preferences, and needs are considered and aligned. You will perform other duties as assigned. You will practice and adhere to the "Code of Conduct" and "Mission and Value Statement. What qualifications you will need: BSN Required Registered Nurse 3+ years in clinical nursing required 2+ years case management required Interqual experience preferred
