Conducts initial therapy assessments and periodic reassessments and documents and reports changes in a patient's condition consistent with Alivia Home Health policies.The Occupational Therapist will provide care in the following counties near Gainesville, FL- Hamilton, Suwannee, Columbia, Union, Bradford, Alachua, Putnam, Lafayette, Dixie and Levy.*Qualifications and Requirements:**Education/Training/Certifications:*· Current Florida Occupational Therapy license· Current Florida driver's license with proof of active auto insurance· Graduated after successful completion of an occupational therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc.
